Crazy Eyes: Adventures in Eye doctors doing breast surgery part deux

While it's hard to top the debacle I wrote about in Atlanta where an eye surgeon nearly boxed someone trying to do a breast augmentation surgery in his office, I came across this other article by another opthomolgist turned "breast expert". I don't know whether he's trying to be clever marketing a procedure or is just ignorant about breast surgery in general, but I came away from this article shaking my head.

The doctor is proposing a breast lift (mastopexy) surgery performed thru the armpit by suturing the breast to the pectoralis muscle and then placing an implant. He describes the surgery appropriate for women with little ptosis. Looking at his picture, you can see that whatever effect he's proposing is nonexistent as the patient doesn't even have ptosis (droop) of the breast. Any illusion of a "lift" is by placing an overly large implant for her frame and actually lowering her breast position to centralize the implant. There is clearly no "lift" going on whatsoever, but rather he's stretched out her lower breast.

I do not predict a good long-term result from this as that skin will frequently continue to stretch unless she develops hardening of her implant. I also would be reluctant to have suggested such a large implant for this patient as you had to violate her native breast boundaries to place it, again a poor strategy for long term results. These are elementary principles of modern breast augmentation.

It's been pretty well established thru collective world-wide experience among plastic surgeons that the maneuver of trying to sew the breast to a higher position to exaggerate the upper pole does not work, which has been demonstrated on a number of follow up studies when this has been attempted.

1. Look at the ear
Try watching and whispering in his ear, because there is no facial plastic surgery that leaves no scar on his ear. Sometimes the transition from the skin of the ear is unclear or there is thickening of the skin due to scarring.

2. It has a "look sinister '
If someone looks good, then suddenly has a face of anger, then it is likely the result of botox. Botox injections in his forehead can distort the eyebrows, Dr. Young called him to one side with "sinister. This can be solved by other injections above the eyebrows.

3. You do not have sagging skin despite being over 60
Dr. Young said that almost all people over age 60 have some sagging skin folds or the skin on the upper eyelid. So if someone can close and open their eyes, then it probably has had plastic surgery.

4. Raisin in the ear
During facelift surgery, surgeons sometimes remove atria shape of the face and pull skin taut and then reattach the ear. If not done properly, then there will be little wrinkles like a raisin in the ear. To fix it, another surgery is needed.

5. The face is swollen
Although rarely found, but there are some people who look younger, but has a swollen face on a globe. This condition can be caused by the injection of excess fat, so it must be treated by liposuction.

6. Bunny Nose
When someone smiles and her nose looks like a rabbit, there is the possibility that Botox injected into the bridge of the nose. This is to get rid of the strange shape of the nose.

7. Do not have a turkey neck
Dr. Young said that almost all people over 50 have skin like a turkey neck. So if you do not, chances are undergoing plastic surgery.

8. Trout Pout

Dr. Young explains the lower lip tends to be 50 percent larger than the upper lip. So if someone has an upper lip that is larger, the possibility that there is a variation of the results of silicone injections.

Breast plastic surgery

If you are someone who has had a mastectomy or have scars on your breasts because of feeding or other problems, then you might be considering breast plastic surgery. While sometimes this can include the use of breast implants, it is also generally used to refer to the complete and total reconstruction of the breast and its components in order to give women more self-confidence. Most often this procedure is seen as something that is therapeutic because it helps to rebuild the self esteem that is sometimes lost when there are problems. After all, most women consider their breasts to be the integral part of what makes them feminine.

One of the most common reasons for breast plastic surgery is after a woman has gone through a mastectomy. A mastectomy is when the breasts are completely removed due to breast cancer. This means that all of the aspects of the breast are removed, including the mound, the areola, and the nipple, and that is what the plastic surgery seeks to rebuild so that these women can feel complete by having a full breast reconstruction. After the battle with cancer many women want to start living a normal life and one way that they can do this is by making sure that they can have breasts again. This is an important part of rebuilding their femininity so that they can move on with their lives.

Another type of breast plastic surgery is when women seek to have their existing breasts enhanced through implants. This too can be therapeutic for some women who feel that their smaller breasts have caused them discomfort and they do not feel full-filled by their current size. Sometimes this means that getting a breast implant can help to boost these women’s self-esteem, and that too can be an important factor. If you are someone who is trying to decide whether or not to have breast implants done, then there are some things you should first take into consideration.

First of all, there are two major types of breast implants that you can choose from, silicone and saline. The silicone implants are filled with a gel that has been manufactured specifically for medical purposes. In the early to mid-nineties there were a lot of complaints that problems associated with the reports of silicone leaking out of the implants and into people’s bodies. This caused the major distributor of silicone implants to go bankrupt in the fact of thousands of lawsuits and several medical investigations were launched. In 1999, however, there was a study done that revealed that there were no serious systemic affects of silicone implants in people.

The distrust of silicone implants caused a rise in the number of patients who were getting the implants that were filled with saline solution instead. Although these implants had been around since the 1960′s, they did not become as prevalent until the restrictions that were placed on silicone implants in the nineties. During this time the shells and the saline implants were continuously updated as the medical understanding of the breast plastic surgery procedures increased. One of the reasons that they were not used as often is because they can cause more wrinkling since they are in liquid form instead of a gel form. For women who are having complete reconstruction done after a mastectomy using saline is not really feasible, instead silicone gel is preferred.

If you are someone who is looking into getting breast plastic surgery done, whether it be for cosmetic reasons or to reconstruct your breasts after a mastectomy, then one of the most important things to remember is that you should consult with your doctor first so that you can go through all of the aspects of the surgery beforehand. This way you can be sure that you are getting what you want from the procedure.

Kate Gosselin: Mommy of Eight to Sexy Mama - cosmetic surgery

Kate Gosselin: Mommy of Eight to Sexy Mama - cosmetic surgery
Since Kate Gosselin first appeared on "Jon and Kate Plus 8"a lot has changed; not just Gosselin's celebrity status but also her body. We have seen Gosselin go from "mommy of eight" to "sexy mama" after a tummy tuck and inception of a fitness regimen. However, many are beginning to speculate that Gosselin's new look may be the result of multiple procedures.
Kate Gosselin: Mommy of Eight to Sexy Mama - cosmetic surgery
For starters, Gosselin appeared on the cover of People Magazine with a great figure and round, perky breasts. It is a known fact that most women lose some of their breast volume after having children. Breasts can begin to demonstrate pstosis or sagging, but not Gosselin's breasts! Though it cannot be confirmed, her breasts may look this good as a result of a breast augmentation and/or mastopexty or lift.

Additionally, rumors have circulated that Gosselin may be an avid fan of Botox, or have undergone liposuction of the submental area of the face (the area below the chin) as her face is taught and skin is smooth. Gosselin may have also undergone other surgical procedures. Our very own Dr. Jennifer Walden says, "Looking at recent photos of her in a bikini, she [Gosselin] may have had liposuction of her thighs as well. In addition, her chin is much more well-defined in recent photos which could be the result of submental liposuction and a chin implant." Whatever you're doing Kate, keep doing it! You look great!

Body contouring with plastic surgery

Body contouring is a general term referring to a wide array of plastic surgeries performed on parts of the body. The goal is to improve the shape and tone of the body part in question, by changing the underlying tissue which supports fat. Body contouring with plastic surgery comprises some of the most common surgical procedures performed on people who’ve lost a significant amount of weight as a way to remove excess skin. It is popular amongst both men and women. Like all surgeries, there are side effects and possible complications. Also, costs can be prohibitive.

Abdominoplasty
An abdominoplasty or “tummy tuck” is a very common cosmetic surgery. Patients with loose skin after pregnancy or individuals with skin sagging after major weight loss will often choose this type of procedure. In general, an abdominoplasty involves cutting from hip to hip just above the pubic area and detaching the skin from the abdominal wall to reveal the muscles and tissue. Excess skin and fat are removed to tighten the area, and the rectus may be tightened. Abdominoplasties can either be “full” or “mini” and can take one to five hours.

Liposuction
Liposuction often accompanies abdominoplasty as a way to further contour the body. Also known as lipoplasty or simply “lipo”, suction-assisted fat removal is a cosmetic surgery operation that can remove fat from a variety of different sites on the body including the abdomen, thighs and buttocks, neck, backs of the arms and elsewhere. It is sometimes used as an alternative to weight loss. During lipoplasty, a cosmetic surgeon removes fat with a cannula (hollow tube) and a suction device. Depending on a number of factors, surgeons vary the amount of fluid used in lipoplasty as well as type of cannula used. Lipoplasty causes bruising and swelling. It may also cause scars, pain, and numbness.

Lower Body Lift
Surgeries to contour bodies have expanded out from the abdomen to include the pubic area, lower back, buttocks and thighs. For example, a lower body lift is another body contouring procedure to reshape a patient’s abdomen, hips, thighs, and buttocks. Someone who has experienced massive weight loss may choose this operation to remove extra skin hanging around the lower torso (also called a belt lipectomy). It is an extension of the tummy tuck, wrapping completely around the lower torso. The surgery can take five to eight hours.

Brachioplasty (Arm Lift)
On the upper body, the shrinking of the arms from weight loss or aging can lead to excess skin hanging from the arm and cause some people to feel self-conscious. Brachioplasty, or an arm lift is used to tighten the excess skin of the arm and armpit. The biggest drawback of the procedure is a long scar in a visible area (the arm). The procedure is sometimes combined with the lifting and contouring of sagging female or male breasts.

Mastoplexy (Breast Lift)

Mastoplexy or breast lift surgery changes the shape of the breast, often to tighten the tissue and prevent visible sagging. It involves cutting into the breast, removing excess skin and suturing the smaller area back together. Various techniques are used to achieve desired results. The surgeon will determine the most appropriate breast lift technique based on various factors such as the desired outcome and the patient’s pre-operative state.

Risks & Costs

Body contouring with plastic surgery will cause scars and no one can perfectly predict how scars will heal on a person. Body contouring surgery can cause a number of minor or major complications. Allergic reactions, and skin and tissue damage and discoloration may occur. Severe complications can include infection, blood clots, thrombosis, cardiac and pulmonary complications. Recovery times can also vary considerably. Body contouring by plastic surgery can also cost anywhere from a few thousand dollars to tens of thousands of dollars and may not be covered by standard health insurance.

Best Candidates
To be a good candidate for these types of surgical procedures you need to be in good health and a stable weight for approximately six months. Also, it is recommended to get medical clearance from your primary care physician.

The Thong Lift?

There is an interesting article in the Daily Mail today announcing:
Bottoms up! The facelift is old hat, the tummy tuck's yesterday's news - the latest craze is the bum lift!
The article goes on to describe the Thong Lift, the name given by a Miami plastic surgeon for fat grafting to the buttocks (a.k.a. Brazilian Butt Lift). Here's more:

The difference between this is and other sorts of fat augmentation is that the surgeon injects fat between layers of muscle, so there is more blood supply to the fat and less of it dies.
The scars left by the incisions needed to inject the fat are hidden, says Mendieta, ‘in the grand canyon where the thong goes’, hence the name of the procedure.

"Grand Canyon?" That's a strange term to use!
I perform fat grafting to the buttocks, but am always concerned about being too aggressive with grafting it into the actual muscles, which have large blood vessels and nerves. This would make me more concerned about a major bleeding episode. The surgeon is definitely well-known for his gluteal work, however, and probably does a few more of these than I do.

Facial Filler Foul Ups Article on MSNBC.com

There is an alarming and interesting article on MSNBC.com regarding the growing incidence of complications with facial filler injections. Facial fillers (like Restylane, Juvederm, and Sculptra) are now the second most common plastic surgery procedure in the country. It's inevitable that as these procedures grow in popularity, the number of people having complications will also increase.

While this article is alarming, it's important for consumers to realize that the most popular injectables (hyaluronic acids like Restylane, Perlane, Juvederm, and Prevelle) have a near-antidote called Hyaluronidase if too much is injected. That's one big reason I prefer these injectables over Radiesse or Artefil. Even so, if you are considering having facial fillers injected, make sure you have it performed in the office of a board-certified plastic surgeon or dermatologist.

Kate Gosselin's Post-Tummy Tuck Belly

The website Poponthepop.com has posted this photo of Kate Gosselin in a bikini, and called it "terrifying" due to the loose skin on her tummy, even after a tummy tuck. What's the scoop here?

Tummy tucks are meant to remove excess, hanging skin (and the fat accompanying it) and tighten up the muscle layer underneath. Although Kate looks flat and tight when standing, it's not surprising that her skin looks this way when sitting. Everyone has some skin that folds over when sitting down. We must, otherwise we wouldn't be able to stand up straight. With Kate, however, that skin has been profoundly stretched out, so it's not going to look as smooth as the skin of a person who has not had children.

So poke fun at her for being an annoying fame junkie, but that tummy skin is completely normal.

E! To Debut New Bridal Plastic Surgery Series "Bridalplasty"

E! has ordered episodes of a new program that mixes a wedding competition with extreme plastic surgery. Bridalplasty combines elements of a game show competition, dream wedding, and plastic surgery documentary. Read about it in the Hollywood Reporter:

Each week, a group of women competes head-to-head in such challenges as writing wedding vows and planning honeymoons. The winner receives the chance to choose a plastic surgery procedure from her "wish list." She's given the procedure immediately, and results are shown at the start of the following week's episode. One by one, the women are voted out by their competitors and, according to the show's description, "possibly walking away with nothing and losing [their] chance to be the perfect bride." The last bride standing will receive a "dream wedding," where she will reveal her new appearance to friends, family and the groom. "Viewers will witness his emotional and possibly shocked reaction as they stand at the altar and he lifts her veil to see her for the first time following her extreme plastic surgery," E! said.

The plastic surgeon from the Swan will be performing all the procedures, and the host of the show is Shanna Moakler.

No, I have nothing to do with this program, but I have heard from a few Hollywood producers that there are a few plastic surgery-related programs currently in the works. This one sounds quite similar to the Swan, which aired several years ago for one full season. I bet there will be a lot of people watching, especially since these brides have got to be absolutely crazy to be featured in a program like this!

Kate Gosselin's Breasts: "These Breasts Are All Mine"

Kate Gosselin's appearance in a bikini has some questioning whether she has breast implants in addition to the tummy tuck she admitted to having. According to RadarOnline.com, Kate has responded to questions regarding breast implants with:

"These breasts are all mine."

So does she have breast implants? I think so. It is unusual for a woman to have given birth to eight children, undergone a tummy tuck, and slimmed down to a tiny size, yet still retain natural, perky, firm, round, and large breasts. Usually when the tummy droops, so do the breasts, although often to a lesser extent. My last quote to Radaronline.com:

Youn quipped that either way, Kate's 'no surgery' claim is accurate: "They are all hers because once she pays for them they are all hers. I've never taken back any patient's implants."

Kim Kardashian Gets Botox! And Has A Bad Reaction Afterwards!

Kim Kardashian has allowed the E! cameras to follow her on an appointment to get Botox. While the procedure appears to go well, she unfortunately suffers from a reaction while at a restaurant afterwards. What could this be from?

There are many possibilities. She could be allergic to the Botox although this is very rare. In fact, my office treats at least 50 Botox or Dysport patients each month, and I've never seen an actual allergic reaction. It could also be due to bruising and swelling causing her discomfort. Finally, and the most likely, maybe she's just a drama queen.

Jane Fonda On Her Plastic Surgery

It's funny that Jane Fonda admitting to having had plastic surgery is suddenly news now, especially since I reported it on this blog 7 months ago (click here for that!). She is now going public with her plastic surgery. According to CBS News:

The 73-year-old actress revealed to Entertainment Tonight that she "recently had plastic surgery."
"It was a hard decision to make, and I decided if I'm going to do it, I'm going to tell the truth," Fonda said. "I'm writing a book about aging, so I can't write that book and not say I've had plastic surgery. And you know, I just decided it was for me -- I don't want to have bags under my eyes that make me look tired, and so forth and so on."

"I got tired of not looking like how I feel and I wanted a more refined chin line like I used to have so I changed my mind."

No one who is 73 looks as good as she does without the help of a scalpel. Yes, that includes you Raquel Welch and Joan Collins! Kudos to Jane's plastic surgeon.

Heidi Montag Tweets Plastic Surgery Advice

Our favorite plastic surgery disaster, Heidi Montag, has now taken to tweeting advice for people regarding plastic surgery. Here are three recent tweets, courtesy of USAToday.com, and my analysis (in italics):

"Warning: to anyone who is thinking about getting their ears pinned, I learned after the fact it is the most painful surgery a person can get" - Untrue. I've performed this surgery many times and pain is almost never an issue. A tummy tuck or breast augmentation is much more painful than an otoplasty (ear pinback).

"Giving my self a soft tissue breast massage. Ladies we have to keep those implants soft" - True. I have all my breast implant patients massage their breasts as a way to help keep the scar tissue and implants soft. Unfortunately, scientific studies don't support this helping, but most plastic surgeons recommend their patients do it anyway.

"I had a nightmare that I slept without my nose tape on thank God it wasn't true!" - Huh? THAT's a nightmare? Geez, my nightmares are more about driving off a cliff or finding myself in school without any pants on.

Has Courtney Love Had a Stem Cell Facelift?

The Daily Mail is reporting that Courtney Love has undergone a stem cell facelift in the Manhattan office of a Facial Plastic Surgeon. Photos show that she certainly looks better than she has in a long time. But are her looks really due to a stem cell facelift?

Stem cell facelifts are definitely the future of facial plastic surgery. The idea behind the procedure is that stem cells are removed from liposuction fat and reinjected into the face. Because stem cells are extremely youthful cells which can transform into new cells in the location they are transplanted, they can theoretically create newer, younger looking skin. At this time, the machines to separate stem cells from fat are not currently FDA approved for cosmetic purposes. Therefore, the closest thing we have to a stem cell facelift in the United States is a 3-D Volumetric Facelift, a.k.a. a facelift with fat grafting. I, along with three other prominent plastic surgeons, introduced this procedure five years ago in the Aesthetic Surgery Journal. Since then, several plastic and cosmetic surgeons have introduced this procedure as their own.

Dancing with the Stars or Dancing with the Plastic Surgery Patients?

Do the casting directors of the new season of Dancing with the Stars like people who've had plastic surgery? It appears so!
Three of the contestants have had obvious plastic surgery, including Florence Henderson (face work), Audrina Partridge (breast implants), and Jennifer Grey (rhinoplasty). I also wouldn't be surprised with David Hasselhoff, Margaret Cho, and Rick Fox have had a minor nip and tuck as well. Plus, host Brooke Burke was married to a prominent Beverly Hills plastic surgeon for years!

For a plastic surgery-related slideshow on these DWTS contestants, click here to be taken to the In Your Face blog.

Has Brad Pitt Undergone Radiesse Injections?

The National Enquirer is reporting that Brad Pitt has undergone filler injections as a way to look younger. According to Celebitchy:
The 46-year-old actor has been getting injections of the wrinkle filler Radiesse to make his face look younger and smoother, the Enquirer has learned. And it appears to be having the desired effect. Not only does Brad’s face look brighter and more radiant, Angelina has given her seal of approval.
“Brad hasn’t looked this good in years - and Angelina is definitely pleased about his makeover…”
For months, the actor hid behind a scraggly beard “because he was so self-conscious about how he was aging.”

I haven't seen photos of him, but a little Radiesse can definitely take off some years. I'd probably recommend a little Sculptra to fill in some of the gaunt areas of his face.
